Integrity Support Coordination provides high-quality, independent NDIS Support Coordination and Specialist Support Coordination for participants with complex needs, psychosocial disability, and high-risk or unstable circumstances.

based in Toowoomba, we support participants, families, and professionals to navigate the NDIS with clarity, structure, and accountability, ensuring supports are implemented in line with NDIS legislation, provider obligations, and safeguarding requirements.

 

Support Coordination (Level 2)

Understand and implement their NDIS plan in line with funding rules

  • Build capacity to make informed, supported decisions

  • Engage providers who meet suitability, safety, and quality requirements

  • Coordinate NDIS, mainstream, and community supports

  • Address service gaps and provider breakdowns

  • Monitor service delivery against agreed obligations

  • Prepare evidence for plan reviews and Change of Situation requests

  • This service is suited to participants who require active coordination and oversight, not just referrals.

  

Specialist Support Coordination (Level 3)

For participants with complex, high-risk, or unstable circumstances, including:
